All bookings must be made no later than 10 days prior to the event on the SMH Booking Form and sent by email only to: firstname.lastname@example.org with final numbers and all dietary requirements to be confirmed only to the same email address no later than 48 hours prior to the event.

The booking form has been redesigned to allow for digital editing, so there is no need to print, handwrite, scan the booking form. All you need to do is download the booking form PDF to your machine document directory, and open the form using Adobe Acrobat Reader which is a free safe PDF reader from Adobe.

This form can be reused again by reopening the last one you filled in, change the menu items, keeping your contact details in there and then file, save as a new file name and then email that new file to firstname.lastname@example.org.
